The 2026 Local Search Checklist for Health & Wellness Businesses
Local search changed shape this year. The same work still decides whether a client finds you, but there is a second audience reading it now: the AI assistants that answer before the links do. This checklist covers both, in the order the evidence says to do them.
Start where the weight is
Whitespark's 2026 Local Search Ranking Factors study, published November 2025 from 47 local search specialists scoring 187 factors, puts Google Business Profile signals at 32% of what decides the local pack — nearly double the next group. Review signals come second at 17%, then behavioural at 13%, citations at 12%, links at 11%, on-page at 10%.
The same panel's estimate for AI search visibility ranks differently: citation signals 24%, Google Business Profile 22%, reviews 19%. Listings matter more to the assistants than they do to the map. That one difference explains why practices with a tidy Google presence still go unnamed by ChatGPT.
1. Google Business Profile, completely filled in
Primary category chosen deliberately rather than approximately. Every service listed. Hours correct, including holidays. Address and phone matching your website exactly. Photos that are actually of your practice. Google states plainly that local results come down to relevance, distance and prominence, and that prominence is partly a function of how many reviews you have.
2. Listings that agree with each other
Your name, address and phone number should be identical everywhere they appear — Google, Yelp, Apple Maps, health directories, your own footer. This is tedious, and it is the highest-leverage AI visibility work available to a local practice, because assistants cross-check sources. BrightLocal's testing across Google AI Mode, Gemini, Perplexity and ChatGPT Search found business websites the most-cited source overall, with Yelp cited in a third of searches. In July 2026 OpenAI licensed Yelp's data to surface inside ChatGPT directly.
3. Reviews with recent dates
Not volume. Recency. BrightLocal's February 2026 survey found 74% of consumers look for reviews from the last three months, and Press Ganey found 64.8% of healthcare consumers consider a review over a year old irrelevant. Ask everyone, ask close to the visit, and never incentivise — Google's policy prohibits it, and the FTC rule that took effect in October 2024 carries penalties of up to $53,088 per violation for incentives tied to positive sentiment.
4. Pages that answer one question each
An assistant can only quote a clear answer. A page that states a real client question and answers it in the next sentence is liftable. A page of philosophy is not. This is where most practices have the furthest to go, and it costs nothing but attention.
5. Schema, with realistic expectations
Ship LocalBusiness and FAQ structured data. It helps machines parse your pages. But a 2026 controlled test across 1,885 pages found adding schema alone produced no measurable lift in AI citations. Schema is hygiene, not a strategy, and anyone selling it as the answer is selling you the easy part.
6. Check both AI surfaces, separately
Google's AI Overviews and Google's AI Mode are different products with different sourcing — SISTRIX found they cite different domains on the same prompt 83% of the time. Check them individually, plus ChatGPT and Perplexity. And check more than once: SISTRIX found 56% of AI Mode's cited sources rotate weekly, with only a stable core of one to five domains persisting across 86.5% of queries. 7. Know what you are aiming at
For health questions, 88% now return a Google AI Overview, per BrightEdge — the highest of any category, up from 72% a year earlier. And BrightLocal found 45% of consumers used an AI tool for a local recommendation in 2026, up from 6% in 2025. This is not a bet on the future. It is the current state of the channel your clients already use.

Do citations still matter in 2026? More than they used to, and for a new reason. They are 12% of local pack weight but the single largest estimated factor in AI search visibility, because inconsistent listings make you a less reliable thing for an engine to quote.
How often should I redo this? Listings and hours, whenever they change. Reviews, continuously. The AI visibility check, monthly — sources rotate too fast for anything less to mean much.
